IgG Test For Herpes - Best Way To Figure Out If You Have Herpes

There are several ways to know if the herpes virus is present in your body. One of the most common ways is herpes blood test.

Herpes blood tests give a confirmation weather you have herpes infection or not. In this test we look for the antibodies developed by the body’s immune system not for the virus. On the basis of antibodies, there are two types of herpes blood tests- IgG and IgM herpes test.

Herpes IgM antibodies are generally detectable after 7-10 days of the infection and the herpes IgG antibodies takes some time of more than 3 weeks. So the herpes IgG antibody test will be accurate after the 4-5 weeks of infection. The Positive herpes IgG test means that you have been infected with the herpes simplex virus and your infection is at least 4-5 weeks older.

When Should You Go For The Test?

If the yellow or grey color blisters or sores appear with the red base on the genitals or face, then it is possible that it may be caused by herpes viruses. So, wait for a month after the outbreak and then go for the IgG herpes test to solve your doubt.

It is also possible that you have an IgG positive and IgM negative test results. But it doesn’t mean that you don’t have herpes. If it happens then discuss this with your doctor and follow his instruction.

Genital Herpes Simplex STD

Genital herpes simplex virus (HSV) infection is primarily a disease of young adults. It is a recurrent, life-long infection. There are two serotypes: HSV-1 and HSV-2. Most genital cases are caused by HSV-2. At least 50 million persons in the United States have genital HSV infection. Sexual encounters are often delayed or avoided for fear of acquiring or transmitting the disease. The psychologic implications are obvious. Herpes simplex virus (HSV) type 2 is not an etiologic factor in cervical cancer as was once suspected.

Most persons infected with HSV-2 have not been diagnosed. Many have mild or unrecognized infections but shed virus intermittently in the genital tract. Most genital herpes infections are transmitted by persons unaware that they have the infection or who are asymptomatic when transmission occurred. The first-episode genital infection may be severe. Herpes simplex infection of the penis, vulva, and rectum is pathophysiologically identical to herpes infection in other areas.

Prevalence From 1988 to 1994, the seroprevalence of HSV-2 in persons 12 years of age or older in the United States was 21.9 percent, corresponding to 45 million infected people. HSV-2 is now detectable in one of five persons 12 years of age or older in the United States.

Genital Herpes Simplex Laboratory diagnosis

The sensitivity of all laboratory methods depends on the stage of lesions (sensitivity is higher in vesicular than in ulcerative lesions), on whether the patient has a first or a recurrent episode of the disease (higher in first episodes), and on whether the sample is from an immunosuppressed or an immunocompetent patient (more antigen is found in immunosuppressed patients). It is crucial to document genital herpes simplex infection in pregnant women and cutaneous herpes in newborn infants. Consequently, in these instances suspicious vesicular and eroded lesions should be cultured. In all other forms the clinical presentation is usually so characteristic that an accurate diagnosis can be made by inspection. A number of laboratory procedures are available if confirmation is desired.

Culture.

The most definitive method for diagnosis in patients who present with genital ulcers or other mucocutaneous lesions is viral culture which can distinguish between HSV-1 and HSV-2. The culture must be obtained from active lesions during viral shedding, which, on average, lasts 4 days. 50% of cultures are negative in recurrent lesions. Vesicles and wet erosions give a higher yield than dry erosions or crusts. Vesicles are punctured and the fluid is absorbed into the swab, which is then rubbed vigorously onto the base of the lesion. Cervical samples are taken from the endocervix with the swab. From the viewpoint of cost, the insertion of separate swabs from a number of anatomic sites (cervix, vaginal ulcers, vaginal fissures, anus) into one culture vial is the most efficient way to collect genital samples for viral culture. Specimens are inoculated into transport media. Results may be available in 1 to 7 days.

Cytology and histology

Cytologic detection of cellular changes of herpes virus infection is insensitive and nonspecific, both in genital lesions (Tzanck preparation) and cervical Pap smears, and should not be relied on for diagnosis of HSV infection.

The histologic picture is characteristic but not unique for herpes simplex. Polymerase chain reaction assays for HSV is available in some laboratories and is the test of choice for detecting HSV in spinal fluid for diagnosis of HSV-infection of the central nervous system.

Genital Herpes Simplex Serology

Serology

Fifty percent to ninety percent of adults have antibody to HSV. Because of the high incidence of antibodies to herpes simplex in the population, assay of a single serum specimen is not of great value.

Subtyping.

There are two serotypes of HSV. HSV-1 infections are primarily oropharyngeal; genital infections can also be caused by this serotype.

HSV-2 infections are primarily genital but are also detected in the mouth.

HSV-1 seropositivity is usually associated with orolabial infection. Herpes simplex virus type 2 seropositivity is usually associated with genital infection. HSV-1 is now a significant cause of genital herpes and is implicated in 5% to 30% of all first-episode cases. The proportion of HSV-1 among initial genital herpes infections is higher among men who have sex with men (46.9%) than among women (21.4%) and is lowest among heterosexual men (14.6%). Receptive oral sex significantly increased the odds that initial infections are HSV-1 rather than HSV-2. Genital HSV-1 may often be acquired through contact with a partners mouth.

Type-specific Serologic Tests

Type-specific and nonspecific antibodies develop during the first several weeks following infection and persist indefinitely. Almost all HSV-2 infections are sexually acquired. Therefore HSV-2 antibody indicates anogenital infection, but the presence of HSV-1 antibody does not distinguish anogenital from orolabial infection. Persons with subclinical or unrecognized infections are best diagnosed by type-specific antibody tests.

Most mothers of infants who acquire neonatal herpes lack histories of clinically evident genital herpes. Many neonatal infections result from asymptomatic cervical shedding of virus after a primary episode of genital HSV in the third trimester. The risk for transmission to the neonate from an infected mother is high (30%-50%) among women who acquire genital herpes near the time of delivery and is low (less than 1%) among women with histories of recurrent herpes at term or who acquire genital HSV during the first half of pregnancy. However, because recurrent genital herpes is much more common than initial HSV infection during pregnancy, the proportion of neonatal HSV infections acquired from mothers with recurrent herpes remains high. Prevention of neonatal herpes depends both on preventing acquisition of genital HSV infection during late pregnancy and avoiding exposure of the infant to herpetic lesions during delivery.

Consider testing pregnant women who are at risk of acquiring herpes in the third trimester. These are women without a history of genital herpes whose partner is known to be infected or is seropositive. Infection of the neonate is most common when the mother develops primary infection late in pregnancy. Transmission of herpes to the neonate during birth results in neonatal herpes, with devastating results.

A pregnant seronegative woman could be identified and counseled to avoid transmission during the third trimester. An HSV-2_eropositive woman can be informed that her risk of transmission to her fetus is low and that most patients can have a vaginal delivery. If a woman is entirely seronegative and her partner is HSV-1 positive, they should be instructed to avoid oral-genital and genital-genital contact in the last trimester, as HSV-1 accounts for up to 30% of cases of neonatal herpes. Consider suppressive therapy for seropositive men unwilling to abstain or to use condoms. "

Approximately 80% to 90% of persons with a symptomatic first episode of HSV-2 genital infection will have a recurrent episode within the following year, compared with 50% to 60% of patients with HSV-1 infection.

Reactivation decreases in frequency over time in most patients. Ninety-five percent of patients with primary HSV-2 have recurrences, with a median time to the first recurrence of approximately 50 days. Fifty percent of patients with primary HSV-1 have recurrent outbreaks, and the median time to the first recurrence is 1 year. Median recurrence rates in the first year are one (HSV-1) and five (HSV-2) per year in patients with newly acquired infection. Patients infected with HSV-2 who were followed for more than 4 years had a median decrease of two recurrences between years 1 and 5. However, 25% of these patients had an increase of at least one recurrence in year 5. Decreases over time among patients who never received suppressive therapy were similar to decreases during untreated periods in patients who received suppressive therapy. Patients who had severe primary infection had recurrences nearly twice as often and had a shorter time to first recurrence compared with those who had shorter first episodes. "

Cathy: Someone wrote in and said, “If you have HSV-1 or Herpes 1, how do you know when to kiss again?”

That’s really a pointy question. If you found out and diagnosed with something, how do you know when you’re safe? How do you know when you can reach out and reconnect again? This is Reid Mihalko from http://www.ReidAboutSex.com.

Cathy: So there isn’t a “safe time” for say where you could not transmit HSV-1. They think it’s likely if you’re not having an outbreak but it’s always important to talk to your potential partners about it before you kiss them. Now, there are lot of people that have HSV-1 and it’s not the…

Reid: I have HSV-1. I have herpes! People still sleep with me.

Cathy: Yes and you tell people before you kiss them? And that’s…

Reid: Well, actually I don’t tell everybody I have herpes before I kiss them.

Cathy: No?

Reid: No. So…

Cathy: You tell everybody before you sleep with them?

Reid: Before I sleep with them. But I’ve made out with lots of people and not told them and… this promise you we get lots of comments about this. You’re going to figure this one out for yourself but what it comes down to is herpes is isn’t fatal and my heart goes out to folks who have herpes that have too many [inaudible 00:01:39] outbreaks.

Cathy: Or compromising immune system before it’s…

Reid: Compromising immune system where it becomes trickier. It’s possible to get herpes in your eye in things like that but for the most parts statistically speaking, herpes is not fatal. Statistically speaking most people have 1, 2, 3 or 4 outbreaks.

Cathy: They get smaller. They get less…

Reid: Outbreaks get less and less. We have better and better medications to help prevent outbreaks and things like that. With those exceptions, herpes is really something that hast just a lot of cultural stigma and baggage. It’s just a pain in the ass.

Cathy: Originally, it was not considered as “sexually transmitted” like issue. There wasn’t really a stigma. It was a skin condition that some people got. I forget it was in a 50s or 70s, one of the drug companies found out something that would help treat it and so they kind of stigmata… if you have this, it’s a sexual, they made it shameful.

Reid: Yeah and depending on your emotional and physical needs are around feeling safe or a partner or a new partner, we have to recognize there’s a lot of stigma here. My advice is for you to figure out what your needs are around disclosure and be as good as you can about those things as best as you can. Also do the emotional work and have community around you if you’re HSV positive so that you can work through the stigma and the shame around it so that you can use your words to talk to people more about this. Sometimes, I tell people that I have herpes when I make me out them.

This is a tricky thing to talk about but if you think you have a cold and you’re going to kiss somebody, do you stop and tell them? There’s a little fly.

Cathy: They fell to the grapes.

Reid: No! If you think you have coming down to cold, do you tell somebody you’re going to kiss them or not? A cold is the most common sexually transmitted infection out there. I want to underscore get good at being able to talk to people about this stuff so that you can speak up and that’s not the issue. To answer the actual question, now we know about herpes that you can be what’s called “shedding”, you can be contagious anytime, you can be contagious when you’re not having an outbreak. You can be somebody like me who’s never had an outbreak that they know of but still test positive and understanding that, “Hey, you know I can give you herpes and not be in having an outbreak, is that okay with you?” It’s a simple check in like that.

Cathy: And you can also check with your doctor. There’s a medicine that helps suppress that… suppress the outbreaks and intentionally reduce the chance…

Reid: Intentionally reduce shedding but I don’t know that…

Cathy: And neither of us are medical doctors.

Kissing is not necessarily a safe act to anybody. We can get a cold, a flu. Aunt Gertrude can kiss us on the cheek with a cold sore and all of the sudden we have HSV-1. It’s not “safe” like it’s never like, “Oh it’s a hundred percent safe to kiss anybody.” But it’s usually worth it.

Reid: And when you are having an outbreak, we know you’re shedding and so then it would not be “safe”, that is when you are contagious for sure and so please avoid kissing people when you have an active outbreak. That’s just common sense but not really common sense because people don’t talk about like that.

Genital Herpes - Know About Your Disease In Less Than 1 Min

Genital herpes is a sexually transmitted infection. It is caused by the herpes simplex virus.

#Causes:-

Genital herpes affects the skin or mucous membranes of the genitals. The virus is spread from one person to another during sexual contact.

There are 2 types of herpes simplex virus (HSV):

HSV-1 most often affects the mouth and lips and causes cold sores or fever blisters. But it can spread from the mouth to the genitals during oral sex.

HSV type 2 (HSV-2) most often causes genital herpes. It can be spread through skin contact or through fluids from the mouth or genitals.

You may become infected with herpes if your skin, vagina, penis, or mouth comes into contact with someone who already has herpes.

You are most likely to get herpes if you touch the skin of someone who has herpes sores, blisters, or a rash. But the virus can still be spread, even when no sores or other symptoms are present. In some cases, you do not know you are infected.

#Symptoms:-

Many people with genital herpes never have sores. Or they have very mild symptoms that go unnoticed or are mistaken for insect bites or another skin condition.

If signs and symptoms do occur during the first outbreak, they can be severe. This first outbreak most often happens within 2 days to 2 weeks of being infected.

General symptoms may include:

Decreased appetite

Fever

General sick feeling (malaise)

Muscle aches in the lower back, buttocks, thighs, or knees

Swollen and tender lymph nodes in the groin

Genital symptoms include small, painful blisters filled with clear or straw-colored fluid. Areas where the sores may found include:

Outer vaginal lips (labia), vagina, cervix, around the anus, and on the thighs or buttocks (in women)

Penis, scrotum, around the anus, on the thighs or buttocks (in men)

Tongue, mouth, eyes, gums, lips, fingers, and other parts of the body (in both genders)

Before the blisters appear, there may be tingling, burning, itching, or pain at the site where the blisters will appear. When the blisters break, they leave shallow ulcers that are very painful. These ulcers crust over and heal in 7 to 14 days or more.

#Treatment:-

Genital herpes cannot be cured. Medicines that fight viruses (such as acyclovir or valacyclovir) may be prescribed.

These medicines help relieve pain and discomfort during an outbreak by healing the sores more quickly. They seem to work better during a first attack than in later outbreaks.

For repeat outbreaks, the medicine should be taken as soon as tingling, burning, or itching begins, or as soon as blisters appear.

People who have many outbreaks may take these medicines daily over a period of time. This helps prevent outbreaks or shorten their length. It can also reduce the chance of giving herpes to someone else.

Side effects are rare with acyclovir and valacyclovir.

Pregnant women may be treated for herpes during the last month of pregnancy to reduce the chance of having an outbreak at the time of delivery. If there is an outbreak around the time of delivery, a C-section will be recommended. This reduces the chance of infecting the baby.

#Prevention:-

If you have genital herpes, you should tell your partner that you have the disease, even if you do not have symptoms.

Condoms are the best way to protect against catching genital herpes during sexual activity.

Use a condom correctly and consistently to help prevent spread of the disease.

Only latex condoms prevent infection. Animal membrane (sheepskin) condoms do not work because the virus can pass through them.

Using the female condom also reduces the risk of spreading genital herpes.

Although it is much less likely, you can still get genital herpes if you use a condom.

People often throw around outrageous, probably inaccurate STD statistics as cautionary tales. But the World Health Organization (WHO) just released a stat that is really turning heads.

More than two-thirds of all people younger than 50 are infected with herpes simplex virus type 1 (HSV-1), WHO estimates in a new report. That's 3.7 billion people.

Now, HSV-1 isn't the one that usually causes genital herpes — that's HSV-2. HSV-1 is the one usually found above the belt, the one that causes "cold sores" near the mouth.

But HSV-1 can cause genital herpes. According to WHO, an estimated 140 million people between 15 and 49 years old have genital HSV-1 infections. Both types of the herpes virus can be transmitted through different kinds of sex, and both are incurable.
